Advanced Care Practitioner - Team Leader
Primary Duties as Team Leader
Clinical Leadership
- Ensure the Home Visiting Team delivers high-quality clinical care, including assessment, diagnosis, treatment, and prescribing, at all times.
- Support complex clinical decision-making.
- Lead on clinical supervision, medication safety, and prescription audits with input from frailty GP and pharmacists.
- Provide support with clinical queries and escalation.
Team Management
- Oversee daily service operations and rota coordination for the AVS team, working closely with Care Coordinators and PCN Leadership Team.
- Manage team development including monthly 1:1s, clinical supervision, probation reviews, annual appraisals, and performance management.
- Lead recruitment and induction of new team members.
- Lead team clinical development and training, ensuring compliance with mandatory training and clinical competencies.
- Foster a supportive and inclusive team culture.
- Develop and implement robust CQC compliance processes.
Service & Strategic Development
- Support development and implementation of Home Visiting Service protocols and policies.
- Collaborate with PCN Business Manager, Frailty GP Lead, and Clinical Directors to enhance service delivery.
- Monitor performance and undertake quality improvement initiatives.
- Ensure delivery of PCN DES, Home Visiting Service, and Proactive Care Service Contracts and QOF targets.
Advanced Care Practitioner Duties
- Use expert knowledge and clinical skills to deliver holistic care to housebound patients and care home residents registered with Southampton West PCN.
- Provide safe, evidence-based, cost-effective, individualised patient care.
- Assess, diagnose, and treat patients with various acute, non-acute, and chronic conditions.
- Refer patients to appropriate agencies and admit to secondary care when necessary.
- Interpret diagnostic tests and communicate effectively with patients and carers.
- Contribute to targets under the PCN Enhanced Service and QOF.
- Undertake quality improvement projects and annual reviews, including medication reviews.
- Record clinical consultations accurately, using SNOMED codes and templates.
- Understand safeguarding protocols and referral procedures.
- Prescribe medications safely within legislative frameworks.
- Build positive relationships with multidisciplinary teams and local frailty teams.
Professional Development
- Maintain professional registration and adhere to the professional body’s Code of Conduct.
- Participate in clinical supervision, PCN, and Practice meetings.
- Engage in training and personal development activities, including annual performance reviews.
