Advance Practitioner - Furnace House Surgery
The Role: We are seeking a qualified and experienced ACP capable of working autonomously to:
- Assess, diagnose, treat, and manage patients independently
- Provide high-quality care, including chronic disease management
- Contribute to excellent clinical services and patient outcomes

Main duties of the job
- Independently prioritize, organize, and manage a complex caseload. Formulate differential diagnoses following specialist assessment; provide evidence-based information.
- Maintain competence in clinical skills for advanced practice, including referrals and pathology reports.
- Utilize specialist knowledge to support the multidisciplinary team and provide clinical advice.

Job responsibilities
Job Title: Advanced Clinical Practitioner (also considering Trainee ACP)
Salary: Band 8A/8B or equivalent, depending on experience
Location: Furnace House Surgery
Accountable to: GP Partners / Practice Manager
Role Summary
This role reflects the four pillars of advanced practice: Clinical Practice, Leadership, Education, and Research & Development. Responsibilities vary by experience but include autonomous assessment, diagnosis, clinical decision-making, and health promotion, as well as acting as a non-medical prescriber where applicable.
Main Duties and Responsibilities
Clinical
- Manage a complex caseload independently, formulating differential diagnoses and providing evidence-based information.
- Maintain clinical competence and fulfill an extended scope of practice.
- Support the multidisciplinary team with specialist knowledge and clinical advice.
Management
- Contribute to clinical governance, risk assessment, incident reporting, complaint handling, and hazard notification.
- Implement NICE standards and ensure safe use of equipment and resources.
Communication
- Maintain accurate records and communicate complex information clearly and sensitively to patients and colleagues.
- Participate in team meetings and case conferences, managing conflicting views professionally.
Policy & Service Development
- Support a culture of continuous improvement and contribute to policy development within your expertise.
Training and Education
- Stay current with research, share knowledge, and supervise junior staff as appropriate.
Audit and Research
- Participate in audits and contribute to quality assurance activities.
Personal and Professional Development
- Maintain competence, participate in appraisals, and pursue ongoing professional development.
Other Duties
- Perform additional duties as delegated, with ongoing review of role responsibilities.
Person Specification
Experience
- Proficiency with databases, spreadsheets, and clinical support systems.
- Experience in data reporting, supervision, and autonomous work.
Qualifications
- MSc in Advanced Clinical Practice or equivalent
- Registered Healthcare Professional and Advanced Clinician
