The successful post holder will be responsible for the assessment of patients who become acutely unwell in the community to ascertain their medical needs and formulate a clinical diagnosis for prompt treatment, thereby preventing avoidable hospital admission.
To practice a case management model of delivery for specialist care, including Long Term Conditions and care during exacerbations, by assessing, planning, prioritising, implementing, and evaluating appropriate interventions to meet patients' needs, which may include prescribing pharmacological agents.
Undertake comprehensive healthcare needs assessments of patients, reassessing as appropriate, working flexibly between the acute hospital and community locations.
The role involves providing a wide range of high-quality nursing care to the people of Tower Hamlets, giving meaningful choices to enable them to maximise their health status and minimise the effects of ill health and disability, to lead as independent a life as possible.
Use professional knowledge, judgement, and organisational ability to determine priorities when planning patient care and workloads. Delegate duties to team members according to their skills and competencies.
Enable independence by teaching, supporting, and encouraging patients and carers to participate in their care where possible.
Plan induction programmes for new staff, junior staff, and specialist nursing students.

Salary: £54,320 to £60,981 per annum, including HCA.
