Our team is seeking a highly motivated practitioner passionate about improving the experience of our ageing and frail population. The successful candidate will have knowledge and experience managing patients with high clinical frailty scores and complex care presentations. Excellent communication skills are essential, especially in discussing clinical conditions and anticipatory care planning. The role involves working across organizational boundaries in partnership with other health and social care teams.
