Job Description:

This position is based at Forth Valley Royal Hospital.

We are seeking a highly motivated practitioner passionate about improving the experience of our ageing and frail population. The candidate must have experience managing patients with high clinical frailty scores and complex care presentations. Excellent communication skills are essential for discussing clinical conditions and engaging in sensitive anticipatory care planning. The role may involve working across organizational boundaries and in partnership with other health and social care teams.

The successful candidate will join a team utilizing Comprehensive Geriatric Assessment to screen for frailty and plan multidisciplinary care. Responsibilities include initial screening, prioritizing care, and providing daily medical reviews of frail patients alongside the multidisciplinary team. Strong organizational skills, excellent communication, and a commitment to professional development are required. Shift patterns are variable depending on location.

We may also consider applications for trainee posts and support development to meet advanced practice competencies through academic courses, supervision, and clinical practice. Applicants for trainee posts should demonstrate relevant coursework for the advanced practice pathway.

This position is also considered as an in-training role under Annex 21 of the Agenda for Change terms & conditions.

Essential Qualifications for Band 7:

  • HEI-accredited Clinical Examination & History Taking Course

Highly Desirable:

  • Previous advanced practice experience
  • Experience practicing autonomously within a frailty setting
