Adult Social Worker

£38.00 per hour

Location: Lincolnshire

Hours: 37 hours p/w, hybrid

As a dedicated social worker, you will play a crucial role in providing statutory professional social work services as part of a team supporting adults with care and support needs. Your commitment to strengths-based practice will be evident through your experience in helping individuals identify and build support networks to achieve their desired outcomes. You will be responsible for assessing, planning, implementing, and reviewing care and support packages for a varied and complex caseload. Additionally, you will offer advice, support, and mentoring to other Adult Care practitioners, demonstrating autonomy in managing your caseload and integrating your practice with colleagues across the health and care sector. This role may involve travel outside the county.

Main Duties:
  1. Manage a varied and complex caseload with supervision and advice from the Lead Practitioner, ensuring accountability for your professional practice.
  2. Conduct needs assessments on complex cases in line with the Care Act 2014, promoting a strengths-based approach to identify care and support needs, desired outcomes, opportunities for independence, and solutions.
  3. Collaborate with specialist safeguarding colleagues to contribute to and undertake safeguarding enquiries, ensuring the person and their desired outcomes are central to safeguarding practice.
  4. Deputize in the absence of the Lead Practitioner, representing them at multi-disciplinary meetings.
  5. Supervise up to four non-registered social care practitioners, including planning, allocating, and checking work, setting targets, addressing day-to-day capability and conduct issues, providing basic staff welfare and support, and identifying training needs.
  6. Supervise and mentor students or act as a Practice Educator for Social Work students, and mentor newly qualified social workers during their Assessed and Supported Year in Employment.
  7. Identify non-eligible and non-social care needs, such as healthcare or housing, and offer information and advice while working with partners to address these wider needs.
  8. Develop value-for-money care packages in collaboration with individuals, carers, and contributing agencies, ensuring eligibility criteria are met, risks are minimized, and quality of life is improved or maintained.
  9. Work with individuals, families, carers, and communities to help them make informed choices and decisions, enabling them to clarify and express their needs and contribute to service planning.
  10. Meet identified needs through informal support and resources available in the public, private, and voluntary sectors.
  11. Research available resources and services for people in need or at risk, identifying and reporting any gaps in service provision.
  12. Operate within delegated budget support mechanisms as they are developed.
  13. Maintain and update case notes and other records, write reports as required, and provide evidence in court in relation to care proceedings if necessary.
  14. Adhere to Lincolnshire County Council Adult Care policies and procedures, including safeguarding policies and procedures.
  15. Focus on specific areas of work as needed based on organizational requirements.
What you need:
  • A degree in Social Work or equivalent
  • Registered with Social Care Wales/ Social Work England
  • Completed relevant post-qualifying training
  • Enhanced DBS check
  • Excellent communication and written skills
