Responsibilities
  • Conduct assessments and reviews using strength-based approaches to support young adults transitioning to independence.
  • Collaborate with a range of agencies including children's services, health, voluntary organisations, and education providers to ensure holistic support.
  • Develop personalised support plans reflecting the individual's needs and goals, and explore all available resources and networks.
  • Facilitate reablement services and the use of assistive technology to enhance independence and reduce risks.
  • Manage and prioritise a caseload, making assessments and interventions where required, including face-to-face visits and telephone enquiries.
  • Apply legal and professional knowledge, conduct safeguarding enquiries and promote self-management among individuals at risk.
  • Maintain professional and accurate records, contributing to reports for legal and service development purposes.
  • Engage in continuous professional development to continuously hone your skills and deliver the best outcomes.
