The ideal candidate for the Social Work role will have several years of experience in a relevant team. You will be experienced in undertaking assessments and timely discharges.
What will your responsibilities be?
As a member of the Hospital Team at Surrey County Council, you will conduct comprehensive assessments, develop and implement individualized care plans, and coordinate timely discharges. You will collaborate with a supportive team, manage a capped caseload, and participate in learning and development opportunities. Your role includes engaging in multidisciplinary meetings, providing support and advocacy for patients and families, maintaining accurate documentation, and ensuring compliance with relevant legislation and policies. Your efforts will promote recovery, independence, and well-being for patients transitioning from hospital care.

Qualifications and Experiences:
You must have strong written and verbal communication skills, a minimum of 2 years of relevant experience undertaking assessments and timely discharges, Social Work England registration, eligibility to work in the UK, a full UK driving licence, post-qualified experience, and a degree level or equivalent qualification in Social Work.
