Job Title: Qualified Social Worker – Adults Hospital Team

Location: Stoke-on-Trent

Rate: Up to £30 per hour

An exciting opportunity has arisen for a Qualified Social Worker to join the Adults Hospital Team. This role is ideal for a dedicated professional aiming to make a significant impact on the lives of adults in a hospital setting. You will be part of a supportive team, working collaboratively to ensure the best outcomes for patients and their families.

Responsibilities:

  • Undertake complex assessments with service users accessing community hospital beds, including Care Act and mental capacity assessments.
  • Develop and implement individualized care plans.
  • Provide emotional support and practical assistance.
  • Coordinate with interdisciplinary teams for seamless transitions.
  • Advocate for patients’ rights and connect them with community resources.
  • Maintain accurate case records.

Qualifications and Experience:

Must be registered with Social Work England and eligible to work in the UK. A full UK driving license and post-qualification experience are required. A degree or equivalent in Social Work is necessary.
