Adults Social Worker – Review Team – Newham, Greater London

Location: Newham, Greater London

Rate: £35 per hour

Job Description:

An exciting opportunity has arisen at the London Borough of Newham Council for an experienced Adults Social Worker to join their Review team.

Responsibilities include:

  • Conduct strengths-based, person-centred assessments to identify the needs and strengths of adults within the community.
  • Deliver exceptional social care to adults and their carers, promoting choice and control.
  • Engage in outcomes-focused conversations, building on the strengths of individuals and their local networks.
  • Conduct regular reviews of care plans to ensure they meet the evolving needs of service users and comply with statutory requirements.
  • Coordinate and deliver integrated care by working with health professionals, housing services, voluntary organizations, and other stakeholders.

Qualifications and Requirements:

  • Social Work England registration and eligibility to work in the UK.
  • Full UK driving licence.
  • Degree-level qualification or equivalent in Social Work.
