Job summary

Sanctuary Personnel is seeking an Adults Social Worker for the Review Team in Tameside. The role offers flexible Part Time / Full Time hours with an ongoing contract, and a chance to support adults to maintain independence and wellbeing within the community.

You will manage varied caseloads, conduct Care Act assessments and contribute to care planning, while working in a supportive, collaborative environment that values professional development.

Qualifications

• Experience managing a varied caseload with personalised, outcome-focused support.
• Ability to undertake Care Act Assessments and reviews in line with legislation.
• Develop and review strengths-based support plans to promote independence.
• Capable of completing Mental Capacity Assessments when required.

Responsibilities

• Manage a varied caseload, ensuring personalised and outcome-focused support.
• Undertake Care Act Assessments and reviews in line with legislation and best practice.
• Develop and review Strengths-based Support Plans to promote independence.
• Complete Mental Capacity Assessments and Best Interest Decisions as needed.
• Signpost to internal and external services to maximise support and engagement.
• Build relationships with health professionals and partner agencies.
