Job summary

5 Star Recruitment is seeking an experienced practitioner to support adults with care and support needs, and carers, enabling personal outcomes in Barry, Wales. The role involves assessments, service planning, co-production with citizens, and partnership work with statutory and voluntary bodies.

You will manage a caseload, maintain electronic records, and engage in supervision and CPD, with potential progression toward Approved Mental Health Practitioner roles.

Qualifications

• Registration with Social Care Wales is required.
• Experience in adult care and support planning is desirable.

Responsibilities

• Undertake assessment of citizens in accordance with statutory duties under relevant legislation.
• Plan and commission services to enable adults with care & support needs and carers with support needs to overcome barriers and monitor care plans through co-production.
• Work in partnership with citizens, carers and adults at risk to provide opportunities for fulfilled lives.
• Negotiate and coordinate applications for resources with internal/external partners, statutory and voluntary.
• Take on case management for a specific caseload and prioritise workload.
• Maintain up-to-date electronic case files and records, providing accurate performance data.
• Prepare for supervision and Personal Development Review and identify training needs to maintain Registration with Social Care Wales.
• Undertake training to support CPD and to enable progression to Approved Mental Health Practitioner or other senior roles.
• Supervise or mentor staff or students where appropriate and contribute to training discussions.
• Participate in office duty rotas and multi-disciplinary team activities.
• Ensure compliance with the Council's Financial Regulations, Standing Orders, Policies and Procedures.
• Apply the principles of the Council’s Equal Opportunity Policy in all duties.
• Adhere to Health and Safety legislation and report safeguarding concerns to the safeguarding officer.
• Support the Environment Policy principles in daily duties.
• Report safeguarding concerns promptly and participate in safeguarding processes as required.
• Perform any other duties commensurate with the grade as directed by the Chief Officer.

MAIN DUTIES AND RESPONSIBILITIES

• To work in partnership with citizens with care and support needs, carers with support needs and adults at risk to provide the best opportunities for people to live fulfilled lives in a way that matters to them.
• To negotiate and co-ordinate applications for resources and to work in partnership with internal and external partners both statutory and voluntary on behalf of citizens and carers
• To undertake case management responsibility for a specific caseload and prioritise workload demands appropriately.
• To maintain up to date electronic - case files and relevant documents, providing accurate records relating to performance management and other statistical data as required.
• To prepare for and participate in supervision and Personal Development Review (#Its About me) sessions with supervisor/ line manager, and identify personal development and training needs to maintain Registration with Social Care Wales.
• To undertake training to support continuing professional development to ensure that the Council is able to retain the provision of its statutory functions, including a willingness for staff to undertake further training to become Approved Mental Health Practitioners, or other more specific, senior roles.
• To supervise or mentor other staff or students where appropriate and offer input into training programmes and Team Practice Issues meetings where appropriate.
• To take part in office duty rotas and other activities associated with a multi-disciplinary team.
• Ensure compliance with the Councils Financial Regulations, Standing Orders, Policies and Procedures
• To implement the principles of the Councils Equal Opportunity Policy whilst carrying out the above duties
• To adhere to Health and Safety legislation / relevant Council policies and procedures and to take reasonable care for the health and safety of yourself and other persons who may be adversely affected by your acts / omissions.

• To report without delay any safeguarding concerns to the appropriate safeguarding officer.
• Such other duties and responsibilities commensurate with the grade and in accordance with the general character of the job as may reasonably be required by the Chief Officer from time to time.
