About the Role

The Home to Assess Service carries out Care Act assessments for adults discharged from hospital, ensuring they receive the right support at home to maintain or regain independence. What We're Looking For

• Qualified Social Worker with SWE registration
• Minimum 2 years post-qualified experience in Adult Social Care
• Full UK driving licence and access to a vehicle
