Role Overview:

Our client is seeking a dedicated and qualified Adult Social Worker to join their team. You will provide support and assistance to adults in need, ensuring their well-being and promoting independence. Collaboration with other professionals is essential to deliver high-quality care and support services.

Your Responsibilities:
  • Manage an adult caseload, perform assessments, and care management tasks under supervision.
  • Conduct and coordinate assessments using Personal Outcome Planning tools with service users, carers, and partner agencies.
  • Develop and implement care packages tailored to individual needs and transitions to Adult Services.
  • Oversee care management to ensure effectiveness and responsiveness.
  • Assist the Team Manager in delivering high-quality services and implementing care plans.

Essential Requirements:

The ideal candidate must have SSSC registration, be eligible to work in the UK, and possess a degree-level qualification in Social Work. Post-qualified experience is also essential.
