Job summary

Redcar and Cleveland Council’s Adults Access Team acts as the front door to Adult Social Care, screening, triaging, and conducting proportionate assessments for incoming referrals. You will gather information, manage risk, promote independence, and ensure timely advice and support in line with statutory duties.

The role focuses on safeguarding, information provision, and coordinating with NHS partners to deliver timely, person-centred care while maintaining clear records and efficient workflow

Qualifications

Adult Social Worker - Adults Access Team

Location: Redcar and Cleveland

Redcar and Cleveland Council are seeking a dedicated individual to join their Adults Access Team. As the front door to Adult Social Care, their Access Team plays a critical role in screening, triaging and conducting proportionate assessments for all incoming referrals.

In this role, you will be at the forefront of gathering key information, managing risk, promoting independence, and ensuring individuals receive timely advice, care, safeguarding interventions, or onward support in line with statutory duties and professional standards.

Key Responsibilities

• Triage and evaluate new contacts and referrals entering Adult Social Care.
• Lead meaningful, strengths-based conversations and proportionate assessments to identify needs, risks, and person-centered outcomes.
• Gather and critically analyse data from individuals, families, partner agencies, and health professionals to inform clear, outcome-focused decision-making.
• Identify and manage safeguarding adult concerns, applying local threshold guidance, assessing risk levels, and recommending Section 42 enquiries when appropriate.
• Apply key statutory legislation effectively, including the Care Act 2014 and Mental Capacity Act 2005.
• Provide quality information, guidance, and signposting to promote independence and prevent the escalation of care needs.
• Collaborate closely with NHS partners, care providers, and voluntary sector organisations to ensure coordinated, timely responses.
• Maintain clear, accurate, and defensible records while supporting daily duty functions to ensure efficient workflow across the team.
