Job summary

Sanctuary Personnel in Middlesbrough is seeking an experienced Adults Social Worker to join the Access Team on an ongoing locum basis. This full-time role offers a daily rate of £283.23 and the opportunity to support adults with a range of needs through assessments and coordinated care plans.

You will work with individuals, families, external agencies and community resources, ensuring high professional standards and person-centred practice while contributing to positive outcomes for service

Qualifications

• Relevant Social Work qualification.
• Current Social Work England registration.
• Experience working within Adult Social Care.
• Strong assessment and care-planning skills.
• Ability to work effectively with adults, families, external agencies, and community services.
• Strong communication, organisation, and professional decision-making skills.
• Ability to manage a varied caseload and prioritise competing demands.
• Commitment to person-centred and strengths-based practice.

Responsibilities

• Conduct comprehensive assessments of adults to identify their needs, strengths, and required levels of support.
• Develop and implement person-centred care and support plans tailored to individual circumstances.
• Liaise effectively with external agencies and community resources to coordinate appropriate and comprehensive support.
• Provide informed advice, guidance, and support to individuals and families experiencing crisis or challenging circumstances.
• Maintain accurate and up-to-date case records in accordance with relevant legislation, policies, and organisational procedures.
• Monitor and review support arrangements to ensure they remain appropriate and responsive to changing needs.
• Participate in regular team meetings to discuss case progress, share knowledge, and contribute to effective team working.
• Work collaboratively with colleagues and partner organisations to promote positive outcomes for service users.
• Ensure all work is carried out in accordance with professional standards and relevant Adult Social Care requirements.
