Transition Social Worker (Children ➜ Adults) – Learning Disability & Mental Health

Work with complexity & variety – assessments, Care Act plans, court reports, Best-Interest decisions… no two days are the same.

What you’ll do
  • Assess & plan: Conduct strengths-based Care Act assessments, Mental Capacity decisions, and develop outcome-focused support plans.
  • Coordinate care: Commission services, monitor delivery, and review changing needs for young people with learning disabilities, autism, and/or mental health conditions.
  • Collaborate: Work closely with CAMHS, SEND, schools, health, and housing to ensure seamless transitions.
  • Safeguard & evidence: Prepare court statements (DoLS, COP DOL, s21a) and provide evidence in court when required.
  • Champion carers: Complete statutory Carer assessments and connect families to appropriate support.
What you’ll bring
  • Social Work degree/DipSW & Social Work England registration.
  • Proven experience with young people with learning disabilities and/or mental health needs, ideally around transition.
  • Knowledge of the Care Act, MCA & SEND/EHCP frameworks.
  • Strong assessment, report-writing, and partnership skills, plus a full UK driving license & car.
