This is a rare opportunity to join a small, supportive social care team based at the Julian Hospital in Norwich. Working as part of a multidisciplinary team (MDT), you will support adults in later life with complex mental health needs across five inpatient wards, including both organic (dementia) and functional (such as bipolar disorder and schizophrenia) services.
Social Workers are aligned to individual wards, providing continuity and specialist social care support throughout a patient's journey, from admission through to discharge planning, review, and community reintegration. The role involves assessment, safeguarding, care planning, Mental Capacity Act work, liaison with families and partner agencies, and supporting person-centered outcomes for patients with complex needs.
We offer a £2000 welcome payment for qualified and experienced Social Workers/Occupational Therapists (grade J and above), Practice Consultants and Team Managers new to Norfolk County Council. Amount to be repaid in full should you leave within two years of employment.
