Qualifications

• Qualified Social Worker with current Social Work England registration.
• At least three years’ post-qualified experience in Adult Social Care.

Responsibilities

• Complete strengths-based Care Act assessments, reviews and support plans.
• Coordinate care and community support to promote independence.
• Undertake complex adult safeguarding work.
• Complete Mental Capacity Assessments and contribute to best interests decisions.
• Support Court of Protection work where required.
• Share your experience and support junior colleagues.

Your role

You’ll support adults with a range of needs, including learning disabilities, physical disabilities, mental health needs and complex social circumstances.

Managing a varied community caseload, you’ll work alongside individuals, families and partner agencies to promote choice, safeguard wellbeing and develop support that reflects what matters to each person.

Your responsibilities will include:

• Completing strengths-based Care Act assessments, reviews and support plans.
• Coordinating care and community support that promote independence.

What you’ll bring
