Job summary

Middlesbrough Council is recruiting a Preparing for Adulthood and Transitions Lead within Specialist and Lifelong Services. If you are a qualified social worker with experience in children’s or adult services and you are looking for something different, this is the role for you.

You will work with the Head of Specialist and Lifelong Services to drive practice changes, coordinate between Children’s Services, Education, Finance, Commissioning and Adult Social Care, identify gaps, advise on

Qualifications

Qualified social worker with experience in children’s or adult services.
Experience in Preparing for Adulthood, transitional safeguarding, CHC alongside knowledge of the Children’s Act and the Care Act is essential.
Ability to work autonomously and with multi‑agency partners.

Responsibilities

Coordinate between Children’s Services, Education, Finance, Commissioning and Adult Social Care to improve outcomes for young people.
Identify gaps in provision and barriers to more independent living.
Undertake transition planning and provide guidance around assessments where eligibility is unclear.
Complete Care Act assessments in preparation for DST CHC meetings with the ICB for young people in their 17th year.
Provide high‑quality, person‑centred social work support for young people preparing for adulthood.
