Adult Transitions Team Social Worker, Peterborough

We are a Transition Team and we support young people from 18 - 25 years completing the Care Assessments, however we start getting to know young people from the age of 16 years. We also complete COP DOLS and MCA's. A person with both Legislative experience of children's and adults will be beneficial to the Team.
