Adult Social Worker, Transitions Team, Birmingham

We are looking for qualified/experienced social workers to work within the Transitions Service.

Day-to-day responsibilities include:

  • Conducting support planning, commissioning of care forms (budget requests), reviews, and mental capacity assessments.
  • Attending meetings such as best interest, EHCP, and section 117 reviews.
  • Completing CHC checklists as part of case management.

You will be fully supported by Senior Practitioners and the assigned Team Manager.

Pay rate: £28-32 per hour
