Job Title: Adult Social Worker

Location: Plymouth

Rate: £32 per hour

An exciting opportunity has recently opened up for an Adult Social Worker to join the team in Plymouth.

Responsibilities include:

• Providing vital support to vulnerable adults as part of a community crisis response team
• Undertaking statutory Care Assessments to identify eligible needs
• Submitting assessments to the panel for funding authorization
• Conducting Mental Capacity assessments

This role is primarily community-based; experience in hospital discharge processes is highly beneficial.

Qualifications and experience:

• Registered with Social Work England
• Degree-level qualification in Social Work
• Minimum of two years post-qualified experience working with vulnerable adults
