Job Title: Adult Social Worker – Preparing For Adulthood Team – Bromley
Location: Bromley
Pay Rate: £37.00/per hour

Liquid Personnel are recruiting a Social Worker for the Preparing for Adulthood Team in Bromley. They will be required to assess individuals, carers, and agencies per Care Act (2014). The role involves initiating and coordinating care and support plans with service users, carers, professional colleagues, and voluntary/private agencies

What will your responsibilities be:

  • Ensuring the ethos of wellbeing is adhered to when working with vulnerable adults and their carers.
  • Participate in the operation of duty and respond accordingly to need, which will involve Safeguarding enquiries and Best Interest Assessment (BIA) when required. Participate in the operation of duty including safeguarding triage and requested allocations.
  • Provide an assessment service to individuals, their carers and other agencies in accordance with Care Act (2014) criteria.
  • Initiate and co-ordinate care and support plans for and with individual service users and support plans for carers, in conjunction with professional colleagues and representatives of voluntary and private agencies.
  • Support staff as agreed with Line Manager and in line with Council procedures.
  • Offer support and advice as appropriate to service users and carers.
  • Through the assessment and prioritisation of need, commission appropriate services and authorise expenditure on services within designated limits.
  • Monitor the efficiency and effectiveness of individual care and support plans.
  • Review the needs of individual service users and carers.
  • Maintain client records in accordance with Council policy.
  • Respond to and where possible resolve complaints within the Council’s complaints procedure.
  • Respond to new and changed legislation in respect of young people and implement as directed.
  • Contribute to service development by providing information and suggestions for future services.
  • Advise team lead on quality and value for money of commissioned services.
  • Participate in multi-disciplinary and inter-agency meetings and working groups and represent the team and service as required.
  • Co-ordinate, participate and where appropriate chair case conferences and planning meetings including safeguarding meetings.
  • Work across teams within both children and adult services as and when needed. The focus of the role is supporting young people 18+. However, as part of planning for a young person’s future, this will need to be initiated from 14 years of age.
  • Undertake any other duties commensurate with the level of the post, as required to ensure the efficient and effective running of the team/service.

Qualifications:

  • For qualified Social Workers, registration with Social Work England
  • Those holding a social work qualification from 2012 onwards must provide evidence of successfully completing an assessed and supported year in employment (ASYE) OR if newly qualified, undertake an ‘assessed and supported year in employment’ as part of the council’s probationary process
