Pay Rate: £32 per hour

About the Role:

Lambeth Council is seeking an adult social worker to join their Neighbourhood Team. The role involves providing support and assistance to adults in the community, ensuring their well-being and access to necessary services.

Responsibilities include:

  • Supporting the home care mobilization project to transfer service users to their neighbourhood providers.
  • Reviewing care needs for adults and communicating with service users and their families about changes in care providers.
  • Ensuring smooth transitions and proper support for service users.
  • Demonstrating excellent communication skills, empathy, and change management to improve service user well-being.
