What you will do:

• Conduct Care Act assessments and reviews to ensure patient needs are accurately identified and met.
• Plan and facilitate discharge meetings ensuring smooth transitions for patients back into the community.
• Complete S117 paperwork, vital for ensuring patients’ rights after leaving hospital care.
• Present cases at the extra contractual panel (ECR) when required, advocating for patient needs.
• Undertake patient assessments and attend weekly ward meetings, providing integral support and collaboration with your team.
