Position: Agency Social Worker (Adult Social Care Reviews)

Note: This is a Long Term Temporary position to start ASAP.

Team Overview:

The Adult Social Care Review Team covers a wide remit of reviews and reassessments across the Wirral, working with persons recently discharged from hospital and those residing in the community. The team supports various adult social care projects, at-risk care home and domiciliary providers, and collaborates with Wirral contracts & commissioning and quality improvement teams, focusing on outcomes and legal literacy. This role offers opportunities for practitioners to strengthen their practice and apply transferable skills in a CPD-focused environment.

Key Responsibilities:

• Supporting service delivery through assessment and intervention skills, framed within legal literacy practices.
• Working with older adults and vulnerable populations.

Essential Criteria:

• Degree in Social Work
• SWE Registration
• Right to work within the UK

Must Have:

• A professional qualification in social work
• Social Work England registration
• Enhanced DBS/Update service
• References for the last 5 years of employment
