Job Title: Adult Social Worker – Learning Disabilities Team
Location: Hammersmith and Fulham
Salary: Up to £35ph

About the Role

We are seeking a dedicated and experienced Adult Social Worker to join our Learning Disabilities Service to provide professional social work services to adults with learning disabilities, mental health needs, or in safeguarding contexts and to promote person-centred care, empowerment, and independence using strength-based and asset-based approaches.

Key Responsibilities

Statutory Assessments: Perform Care Act 2014 assessments (Sections9, 13, 42), including NHS Continuing Healthcare and DoLS considerations.Support Planning & Reviews: Co‑produce and manage personalised care/support plans with service users, families, and carers.Safeguarding Adults: Identify, investigate, and respond to safeguarding concerns, ensuring outcomes focus and adherence to best practice (“Making Safeguarding Personal”).Multi-Agency Collaboration: Work with health services, housing, education, police, and third-sector organisations to coordinate holistic support.Legal & Policy Adherence: Operate within legislative frameworks, including Care Act, Mental Capacity Act, Mental Health Act, and professional guidelines.
