Job summary

Sanctuary Personnel is recruiting an Adult Social Worker for the Initial Assessment Team in Tower Hamlets. The role offers both part-time and full-time options with an attractive hourly rate of £32 per hour and an ongoing contract.

You will conduct fast-paced front-door assessments, complete Care Act and Mental Capacity assessments, and work closely with individuals, families, and partner agencies to plan appropriate support while upholding statutory duties and safeguarding standards.

Responsibilities

• Conduct timely and person-centred assessments of adults who may require care and support services through a fast-paced front door team.
• Complete Care Act assessments, Mental Capacity Assessments, and risk assessments to identify individual needs, strengths, safeguarding concerns, and eligibility for support.
• Collaborate effectively with individuals, families, carers, and partner agencies to develop appropriate care and support plans.
• Promote independence and wellbeing while ensuring statutory duties are met.
• Maintain accurate records and contribute to positive outcomes through successful assessment, intervention, and signposting to relevant services.
