Job Description:

Adult Social Worker, Hospital Team, Slough. The hospital team Pathway involves completing assessments, reviews, DSTs, and panel work. The role requires working in the office on Wednesdays, with travel to Slough to complete assessments. The role is under strict timing to assess patients, enabling them to move off D2A and onto long-term care if appropriate. Knowledge of technology and community work skills are needed, as access to transport is required.

£36.81 per hour.
