Job Title: Adult Social Worker – Hospital Team Newham
Location: London Borough of Newham
Rate: £32.01 per hour

The London Borough of Newham Council is recruiting for a qualified Social Worker to join their Hospital team in Newham.

What will your responsibilities be?

In this role, you will be conducting comprehensive assessments, developing and implementing care plans, and coordinating with multidisciplinary teams to ensure holistic care. You will provide emotional support, advocacy, and practical assistance to help patients navigate the healthcare system and access necessary resources. Your work will involve close collaboration with medical staff, community services, and other stakeholders to promote patient well-being and facilitate safe and timely discharges. This role requires strong communication skills, empathy, and a commitment to improving the lives of vulnerable adults.

Qualifications and Experiences:

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK licence
