Job Title: Social Worker – Hospital Team
Location: Hackney
Rate: £32 per hour
An exciting job role has recently become available for an Adult Social Worker to join a Hospital Team based in Hackney.
What will your responsibilities be?
In this role, you will have a single point of access and use a multi-disciplinary assessment and care planning framework. You will respond swiftly to service user needs and, where possible, treat people in their own home or normal place of residence. You will also provide short-term bed-based intervention as appropriate. This will be in partnership with individuals, carers, professional staff in other agencies, and service providers to establish arrangements and services that meet their assessed needs and promote independence, choice, and control. You will operate within the agreed performance management and budgetary frameworks. You will manage a caseload of individuals and carers who require additional support or guidance to identify their needs and who may need close monitoring or frequent reviews and interventions to minimize and manage risk or where best interest decisions may be required. You will provide assistance, guidance, and direction to colleagues and assist senior colleagues and managers as required. You will adhere to the standards of proficiency set by the Health and Care Professional Council so that registration can be maintained and learning or development needs are duly accounted for in supervision and appraisals.
Qualifications and Experiences:
To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
