Our workforce is our asset. We seek motivated, forward-thinking Social Workers to join our teams, especially within our Home First Hospital Discharge Hub, which prioritizes safe and timely discharges from hospital sites to support Rochdale residents returning home.

What are we looking for?

• Hospital discharge experience with a minimum of two years post-qualification
• Knowledge of the Mental Capacity Act and Mental Capacity Assessments/Best Interest Process
• Understanding of the Care Act 2014 and a strength-based approach to assessment and support planning
• Effective multi-tasker with excellent organizational skills
• Motivated, professional, enthusiastic, and a team player
• Excellent interpersonal, communication, and ICT skills (Word, Excel, Outlook)
• Attention to detail with accurate record-keeping skills
• Access to a car and ability to travel

Important: This is a Long Term Temporary post to start ASAP.

Minimum Requirements:

• Professional qualification in social work
• Social Work England registration
• Enhanced DBS/Update service
• References for the last 5 years of employment
