Job Title: Adult Social Worker – Hospital/D2A

Location: Wolverhampton

Rate: £33.30 per hour

An exciting opportunity has arisen for a qualified Adult Social Worker to join the Hospital/D2A team in Wolverhampton. The pay rate is £33.30 per hour.

Responsibilities:

• Provide professional social work services within the Hospital Social Work Team.
• Support discharge processes and complete assessments under the Discharge to Assess (D2A) model, covering pathways 1, 2, and 3.
• Handle safeguarding enquiries and collaborate with other agencies and health colleagues.
• Assess needs and deliver interventions to safeguard adults, maintaining confidentiality with sensitive information.

Qualifications and Experience:

• Registration with Social Work England.
• Eligibility to work in the UK.
• Full UK driving licence.
• Minimum of 3 years’ experience in social work.
• Experience in multi-agency working.
• Degree level or equivalent in social work.
