Qualifications

• Recognized degree in Social Work or equivalent.
• Current professional registration with the relevant body.
• Strong understanding of adult safeguarding principles and legislation.
• Excellent communication and interpersonal skills with diverse clients.
• Proficient in assessment and care planning processes.

Responsibilities

• Conduct comprehensive assessments of adult clients to determine needs and eligibility for services.
• Develop and implement tailored care plans in collaboration with clients, families, and other professionals.
• Provide ongoing support and advocacy for clients, ensuring their voices are heard in decision-making processes.
• Work closely with multidisciplinary teams to coordinate services and resources effectively.
• Maintain accurate records and documentation in compliance with policies and legislation.
