Job summary

Oxfordshire County Council is seeking a committed adults social worker to join the Adult Community Team in Oxfordshire. This role supports adults to live independently and safely by carrying out strength-based assessments, coordinating support, and collaborating with health partners, families and community organisations.

You will work in a multidisciplinary team, manage a varied caseload, and provide guidance on assistive technology, reablement services and housing adaptations.

Qualifications

• Qualified social worker with Social Work England registration.
• Strong knowledge of Adult Social Care legislation and safeguarding.
• Ability to work independently and within a multidisciplinary team.
• Excellent communication, assessment, and report-writing skills.
• Commitment to professional development and reflective practice.

Responsibilities

• Complete strength-based assessments, reviews, and interventions.
• Support individuals under the Care Act 2014, MCA 2005, and relevant legislation.
• Develop clear, outcome-focused support plans tailored to individual needs.
• Work with multidisciplinary teams, community partners, and families to achieve the best outcomes.
• Provide advice on Assistive Technology, equipment, reablement services, and housing adaptations.
• Manage a varied caseload, including duty responsibilities and community visits.
• Maintain accurate, defensible case records and contribute to service development.
