Please be aware this is a Long Term Temporary post to start ASAP.

Working Age CMHT

undertaking Care Act assessments,

arranging care packages if needed & monitor its effectiveness.

Monitoring MH and supporting people to remain well.

As part of a MDT you will be working with health colleagues to manage risk.

Record notes on Paris system.

The following is essential criteria:

– Degree in Social Work

– SWE Registration

– Right to work within the UK

You must have:

A professional qualification in social work

Social Work England registration.

Enhanced DBS/Update service.

References required for the last 5 years of employment.
