Pay rate - £38 per hour

Contract role, Care Act Assessment Team

Pertemps is recruiting an experienced Adult Social Worker in Herefordshire to work in a Care Act Assessment Team. The role involves carrying out short-term assessments and support planning with service users throughout Herefordshire who have recently left hospital via Discharge to Assess and may require ongoing services.
