What you will do

• Work within the Hayes and Five Elms Locality Team focusing on care management interventions and coordinating annual reviews of support plans.
• Conduct assessments under the Care Act 2014 and support young people transitioning from Children’s services to Adult services.
• Assist Younger Adults with physical disabilities and sensory impairments, as well as Older People, to live independently.
• Collaborate with carers to ensure that risks are appropriately managed.
• Develop and maintain close working relationships with partner agencies.
• Participate actively in multidisciplinary team meetings, demonstrating effective communication and negotiation skills.
