Job summary

Sheffield City Council is seeking a Qualified Social Worker to join the Adult Hospital First Contact Team & Out of Hours, based at the Northern General Hospital. Qualified Social Worker -Adult Hospital First Contact Team & Out of Hours

We currently have an exciting opportunity for a Qualified Social Worker to join our Adult Hospital First Contact Team, based at the Northern General Hospital. This dynamic role combines working within the Care Transfer Hub alongside working out of hours on a rota which includes evening and weekend shifts. The role is office based.

Our team works alongside colleagues from Sheffield Teaching Hospitals to support adults who are hospital inpatients. We play a vital role in facilitating safe, timely discharges by undertaking Care Act assessments, supporting discharge planning, screening safeguarding concerns and providing targeted case management.

This is an excellent opportunity for a practitioner who enjoys variety, autonomy and collaborative working. The role offers the chance to develop expertise in both hospital discharge planning and urgent out of hours social work practice. As part of the Out of Hours service, you will provide a duty response across Adult Care and Wellbeing services.

We are keen to hear from Social Workers who have experience of safeguarding adults and applying relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005. Enjoy working in multidisciplinary settings and building positive professional relationships. Can think creatively and work in a solution-focused way. Are confident making decisions and managing risk in complex and sometimes fast-moving situations. Can work flexibly and effectively under pressure to meet tight timescales.

Experience of duty work, assessment and crisis intervention would be advantageous, although we recognise the value of transferable skills and a willingness to learn. Training will be provided on the specialist IT systems used within the service.

As the role includes evening and weekend out of hours work applicants must hold a full driving licence and have access to a vehicle for these shifts.

If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
