Pay rate £40 per hour, 30 hours per week

Contract role, Hospital Team

Responsibilities

• Lead the Hospital Discharge and Post-Discharge Assessment Team as the Team Leader.
• On-site requirement to be present for 3 days per week across Calderdale Royal Hospital, Huddersfield Royal Infirmary, and Princess Buildings in Halifax.
