Adult Social Care Support Worker - Technology Enabled Care

Fixed Term, Full Time

Location: Epping

Working Style: Community-based worker

Please note that this is a fixed term contract / secondment opportunity for a period of 12 months.

ECC Adult Social Care is making a transformational shift from a focus on long-term care support, to a model with four key principles: prevention, early intervention, enablement and safeguarding. Enabling people to access information and tools, to live independently and allowing a positive response to growing service demand within the financial constraints of ECC.

The Discharge to Assess model of practice is evolving, and as a team member, you will influence change and progression. You will support adults discharged from hospital or those at risk of admission, facilitating their safe return home and supporting their independence.

Where possible, adults will be supported to return to their homes for assessment. Support will focus on enabling independence in their own homes. Some adults may be discharged to temporary care environments for recovery and assessment to determine their care needs.

Responsibilities:

  • Engage with acute partners to raise awareness of Technology Enabled Care (TEC).
  • Identify adults who could benefit from TEC on wards.
  • Collaborate with Transfer of Care (TOC) team to review cases before discharge for TEC consideration.
  • Complete ordering processes including CARES, Mosaic contact form, and Benefits Realisation.
  • Attend Digital Champion meetings on behalf of the hospital.
  • Participate in Fortnightly Ops calls to discuss issues and progress.
  • Coordinate with TEC team and Millbrook regarding TEC options and OT assessment requests.
  • Gather case studies and share best practices.
  • Share knowledge with hospital colleagues to promote TEC awareness.
  • Identify opportunities to integrate TEC into care pathways.
  • Test and learn new technologies.

Skills, Knowledge, and Experience:

  • Experience in social care; NVQ/BTEC Level 3 or equivalent preferred.
  • Continuous professional development and relevant knowledge.
  • Ability to build effective relationships with adults, families, and carers in challenging situations.
  • Experience with activity-based work with adults.
  • Knowledge of relevant social care legislation and policies.
  • Proven ability to collaborate with partners and external agencies.

This role requires mobility across the operational area. A driving licence and access to a vehicle, or other mobility means, are necessary.
