Adult Social Care Support Worker - Technology Enabled Care
Fixed Term, Full Time
Location: Epping
Working Style: Community-based worker
Please note that this is a fixed term contract / secondment opportunity for a period of 12 months.
ECC Adult Social Care is making a transformational shift from a focus on long-term care support, to a model with four key principles: prevention, early intervention, enablement and safeguarding. Enabling people to access information and tools, to live independently and allowing a positive response to growing service demand within the financial constraints of ECC.
The Discharge to Assess model of practice is evolving, and as a team member, you will influence change and progression. You will support adults discharged from hospital or those at risk of admission, facilitating their safe return home and supporting their independence.
Where possible, adults will be supported to return to their homes for assessment. Support will focus on enabling independence in their own homes. Some adults may be discharged to temporary care environments for recovery and assessment to determine their care needs.

This role requires mobility across the operational area. A driving licence and access to a vehicle, or other mobility means, are necessary.
