Job summary

The post is located within the Adult Services Review Team, based at County Hall, and will be accountable to the Team Manager. The post holder will work closely with the Team Manager, Social Workers and Duty Workers to coordinate services and ensure care plans are followed.

You will arrange services, provide practical support, participate in team duties and meetings, and undertake ongoing training to contribute to service development within the council.

Responsibilities

• Arrange and monitor appropriate services in line with individual care plans.
• Provide advice and practical help.
• Liaise with voluntary and statutory agencies.
• Complete annual care plan reviews and reassessments.
• Participate in team meetings and duty rotas.
• Undertake these duties in line with Council Policies and Procedures.
• Undertake training and self-development and contribute to this process in others.
