Adult's Social Worker, Hospital team, Wiltshire

Pay rate: £36.65 per hour

Contract role, Hospital team

Pertemps is recruiting for an experienced Adult's Social Worker in Wiltshire to work within a Hospital team.

Background

• The project aims to facilitate early discharge planning through early identification of discharge pathways, gathering collateral history from multiple sources, and supporting decision-making prior to NC2R.

• It involves setting expectations for patients, staff, and partner organizations regarding discharge pathways and resources.

• While the number of patients with no criteria to reside may not decrease, the delay days are expected to reduce.

• Outcomes are measurable, and any unintended consequences will be reviewed as the project develops.

• The project will demonstrate a model that can influence discharge services across SFT NHS FT upon review.

• A small team of experienced staff from Wiltshire Council and SFT will undertake early assessments of patients over 65 admitted within 48 hours, to establish discharge plans, identify complexities, and initiate background work within the Trust or community, prior to the patient becoming NC2R and within their EDD.

• This approach aims to decrease post-NC2R delays and provide data for further development.
