Adult’s Social Worker, Hospital Team, Bristol

Pay rates: £32 per hour

Contract role, Discharge to Assess

Pertemps is recruiting experienced Adult’s Social Workers in Bristol. This role involves working in a Hospital Team at the Bristol Royal Infirmary and Southmead Hospital.

The Role:

1. As a social worker in Adult Social Care, you'll play a crucial role in assessing and addressing the diverse needs of our community. Your responsibilities include:
2. Conducting assessments and developing tailored care plans for service users and carers, ensuring their unique needs and preferences are met.
3. Advocating for choice and control, empowering individuals to make decisions and manage risks, and organising services in line with their needs.
4. Maintaining professional standards as the service evolves, monitoring changing needs, developing support plans, utilising resources, and incorporating assistive technology to keep Social Care future-ready.
