Adult’s Social Worker
Team: Hospital

Details:

We are currently recruiting seven Senior Social Workers to support the Home First and Hospital Discharge Neighbourhood Teams (HDNT). These roles are essential in delivering statutory adult social care duties, including assessments, reviews, and interventions. Our approach is home-first and strengths-based, and we are particularly keen to hear from social workers with significant experience in hospital discharge work. As a Senior Social Worker, the successful candidate will:

  • Hold a caseload of complex cases
  • Carry out person-centred assessments of service users and carers
  • Work in line with statutory and directorate procedures
  • Communicate effectively with service users, carers, colleagues, and partner agencies

Requirements:

  • A recognised social work qualification (BSc, BA, MA, DipSW, CQSW, or equivalent)
  • Proven ability to manage and prioritise a complex workload
  • Strong written and verbal communication skills
  • Ability to share information clearly and concisely across a range of stakeholders
  • Contract length: 16 weeks
