Adult’s Social Worker
Team: Hospital
Location: South Lakes Kendal

Details:

We are looing for passionate and dedicated Social Worker, with a strong foundation in professional practice backed by a Social Work Degree and registration with Social Work England. You will have experience in independent assessment and support planning, alongside a good working knowledge of current legislation, theoretical models and best practice approaches.

There will be a requirement to work collaboratively within the Transfer of Care Hub for Multi-disciplinary discussions, and for Social Work intervention during hospital admission in order to facilitate and support strength-based Hospital discharge plans.

To work in the community undertaking strength-based assessment and advocating and supporting people after discharge form hospital.

  • DIPSW or equivalent qualification in Social Work
  • Social Work England registration
  • Enhanced and in date DBS certificate.
  • Excellent communication skills and be able to make good decisions based on information available.
  • Can work alone and within/contribute to a team/multi-agency set ups
  • Possess knowledge of service needs.
  • Competent IT skills and be able to navigate electronic file/recording systems.
  • Can write clear, concise, and robust assessment/reports/files.
  • Have a flexible approach to work and working hours when the need arises
