Pay rate to £36 per hour
Contract role, Community Response Team

Pertemps is recruiting for an experienced Adult's Social Worker in West London.
You will work in a Community Response Team.

• The role holder will assist individuals, carers, and families to develop support plans to meet the outcomes identified, using a strength and asset-based approach and family, friends, and community options where appropriate and eligible needs are identified.
• They will actively encourage involvement and ownership of the support plan but take a leadership role where appropriate, and will give clear explanations of the use of Personal Budgets and Direct Payments so that informed decisions can be made.
