Job Title: Adult Nurse Case Manager (Tracheostomy Experienced)

Location: Wimbledon, Head Office Base

Working Hours: Monday to Friday, 9am to 5pm

Job Description:

At HFH Healthcare, we provide specialist nurse-led services to adults and children in their homes across London and the Southeast. We partner with NHS teams to deliver complex care, enabling clients to stay at home even with highly complex health needs. We seek an Adult Nurse Case Manager (Tracheostomy Experienced) to support clients and their families through face-to-face care and personalized care planning. You will work closely with carers, training and supporting them with clinical skills.
