Job summary

Real Life Options is seeking a Support Worker (Practitioner) in Dunfermline to support adults with learning disabilities, including those transitioning from hospital to community settings. You will provide physical and emotional support, enabling independence and meaningful daily activities within a person-centred approach.

Qualifications

• Experience supporting adults with a learning disability and behaviours that may challenge in health or social care settings.
• Demonstrable experience in crisis intervention and positive behaviour management.
• Experience getting to know people individually and ensuring their needs are met in a caring, kind and respectful way.
• Experience working with MDT teams and participating in complex case reviews for people supported.
• Experience working independently.
• Experience in complex care within a social or health care setting.
• Experience with people who present behaviours that require intensive support planning and management to ensure safety and quality of life.

Responsibilities

• Promote positive risk taking while adhering to health and safety legislation.
• Encourage health and wellbeing including support with medications and personal care as needed.
• Adhere to organisational recording and documenting systems.
• Communicate with external agencies and stakeholders professionally.
• Attend mandatory training and team meetings.
• Work flexibly to meet the needs of the people supported (days/evenings/weekends/bank holidays).

Education

SVQ Level 3 Health and Social Care (Scotland) or Level 4 Diploma in Care (England)
